The role of this position is to promote and support an individual's independence, community integration, personal development, social interactions, and work opportunities as part of RehabCare services. There may also be the opportunity to provide support for individuals with Autism or physical and sensory disabilities.
Key Responsibilities- To develop and deliver tailored community support packages that meet the needs, choices, and abilities of service users.
- To plan, implement, and review educational, recreational, and occupational programs within the community in line with best practice and up-to-date research.
- To collaborate with families, multidisciplinary teams, and community-based personnel to facilitate accessible community activities for service users.
- To ensure all interactions with service users are approached with dignity, respect, equality, and choice.
- QQI level 5 in Health & Social Care or equivalent qualification.
- Previous experience working in the Community, including 1:1 support.
- Understanding and knowledge of intellectual, physical, and/or mental health disability.
- Full Clean Driving Licence and access to a car for business purposes is required.
- To promote and contribute to teamwork at all times.
- To maintain a basic knowledge of first aid/personal care and adhere to good hygiene practices.
