The Contributor is seeking a full-time Hospital Liaison to join our SOAR team on a year-long, grant-funded project aimed at supporting individuals experiencing homelessness, chronic health conditions, and high emergency department usage at local hospitals. This role involves engaging clients in person at local hospitals and in the community, assisting them in obtaining disability benefits (SSI/SSDI), and connecting them to vital resources such as housing, healthcare, and other supports.

This position allows for some remote work, but most of the time is spent locating and engaging clients in person. We prioritize candidates with lived experience of homelessness or mental health challenges, who bring empathy, resilience, and a commitment to making a tangible difference in people’s lives.

Job Responsibilities

Outreach and Engagement

  • Engage individuals in emergency departments, encampments, and community settings, building trust and rapport.
  • Partner with clients to access disability benefits, housing, and other essential supports.
  • Use motivational interviewing techniques to conduct respectful, goal-oriented conversations.
  • Leverage personal lived experience to foster hope and empathy.

Care Coordination and SOAR Activities

  • Collaborate with healthcare providers and nonprofit partners to coordinate referrals and care.
  • Conduct interviews to gather personal narratives, medical histories, and signed releases of information.
  • Assist clients with SSI/SSDI applications and collecting medical evidence for their claims.
  • Transport clients to appointments using personal vehicles, as needed.
  • Document client interactions and progress accurately and confidentially in line with organizational policies.

Housing-Focused Advocacy and Client Support

  • Support clients in defining and pursuing their personal goals for housing, health, and well-being.
  • Navigate housing applications and processes to help clients gain access to housing.
  • Educate clients about mental health, homelessness, and available resources.
Qualifications
  • Lived Experience: Experience with the mental health and homeless systems through personal experience.
  • Certification/Training: Certified Peer Recovery Specialist (or willingness to obtain) and SOAR training preferred. Motivational Interviewing training is a plus.
  • Work Experience: Prior experience in peer support, care coordination, or advocacy preferred.
  • Education: High school diploma or equivalent; additional social work or psychology education is a bonus.
  • Skills: Strong interpersonal skills, empathy, and familiarity with local homeless and health resources.
Additional Information
  • Full-time, in-person (with some remote flexibility), Monday through Friday (9 am – 5 pm).
  • Salary: $45,000, including 100% paid individual medical insurance, dental, vision, short and long-term disability, life insurance, 403(b) contributions, and generous paid time off.
