Currently, we have an opportunity for a passionate Care Coordinator in Sligo. Based in our Sligo office, this person will work with a great team to provide client support services in the northwest region.

If you have experience working in a care environment or have strong customer service skills and are looking for an opportunity to progress your career, then this opportunity may be for you. Full training in the role will be given.

Key Responsibilities:
  • Communicate effectively with the HSE re homecare packages and coordinate services for clients
  • Accept, allocate and process referrals for new work in accordance with carer capacity in an area
  • Organise carer & client work schedules that maximise capacity
  • Liaise with clients and their representatives about their care and offer support when required
  • Work closely with managers, team leaders and carers to optimise care delivery
  • Monitor the delivery and quality of care; follow-up on care concerns and produce monitoring reports
  • Promote the principles of person-centred care ensuring clients are treated with dignity and respect
