Responsibilities:
Conduct comprehensive psychosocial assessments and develop individualized care plans for older adults.
Provide therapeutic interventions and support to patients and their families.
Collaborate with multidisciplinary teams to deliver holistic care.
Advocate for patients' rights and access to services.
Maintain accurate and up-to-date case records and documentation.
Participate in team meetings, case conferences, and training sessions.
Requirements:
Minimum of a level 8 degree in Social Care.
3 years post qualifying experience to work at Senior Social Worker level 8.
CORU registration.
Experience in mental health services, preferably with older adults.
Strong assessment, intervention, and case management skills.
Full driving license and access to own car is desirable.
Excellent communication and interpersonal skills.
Resident in Ireland and legal right to work in Ireland
without restriction
.
All candidates will be required to undergo Garda Vetting.
International Police Clearance (if lived outside of Ireland for more than 6 months after the age of 16).
