high-quality, person-centred care toindividuals in their homes.This role involves responding promptly to urgent care needs, offeringsupport to regular care assistants, and ensuring continuity of care.ResponsibilitiesEmergency Response:Respond promptly to urgent calls and provide immediate care and support to customers.Assess client needs quickly and efficiently to deliver appropriate care interventions.Communicate effectively with the care management team to report on the situation andactions taken.To adhere to existing working practices, methods, procedures, and any other specificapproaches as layout within the care plan so as not to cause any confusion for the customer inproviding short-notice care.Personal CareAssist customers with daily living activities such as bathing, dressing, grooming, and toileting.Support customers with mobility, ensuring safe transfers and use of equipment wherenecessary.Prompt medication as prescribed, following company policies and procedures.Ensure each task performed with the Customer (personal care, meal preparation, etc) iscompleted with dignity and respect for the Customer.Emotional And Social SupportProvide companionship and emotional support to customers, encouraging social interactionand participation in activities.Assist with meal preparation and household tasks to ensure a safe and comfortable livingenvironment.Coordination And CommunicationWork collaboratively with other healthcare professionals, family members, and caregivers toensure comprehensive care.Maintain clear and timely communication with the care management team regarding anyconcerns or updates on client care.Works together with the Branch manager and Care Coordinator teams.QualificationsQQI Level 5 Health care
