Key Responsibilities:
Collaborate with the young person, social workers, carers, and others to assess needs before age 18
Develop a personalized
Aftercare Plan
six months prior to the young person's 18th birthday
Become the young adult's
main support person
once they turn 18, replacing the social worker
Provide guidance on housing, education, employment, budgeting, health, and relationships
Represent the young person in accessing services like mental health, social welfare, and housing
Requirements:
Bachelor's degree in social care practice (Applied social studies in professional social care - Level 7 or 8 degree)
CORU Registration
Must be a resident in Ireland and have the legal right to work in Ireland without restriction.
Willing to undergo Garda Vetting.
International Police Clearance required (if lived outside of Ireland for more than 6 months after the age of 16).
