PE Global Healthcare are recruiting for agency social workers & social care workers in Kerry Key Responsibilities: Client Assessment: Conduct comprehensive assessments of individuals and families to identify their needs, strengths, and challenges.

Care Planning: Develop and implement individualized care or intervention plans in collaboration with clients and other professionals.

Counselling and Support: Provide emotional support, crisis intervention, and counselling to individuals or families facing challenges such as trauma, abuse, addiction, or mental health issues.

Resource Coordination: Connect clients with appropriate resources, including social services, healthcare providers, housing, financial aid, and employment support.

Advocacy: Advocate on behalf of clients to ensure they receive appropriate services and support, while protecting their rights and dignity.

Case Management: Monitor and document client progress, adjusting care plans as necessary.

Maintain accurate and confidential case files.

Collaboration: Work closely with other social workers, healthcare professionals, educators, law enforcement, and community organizations to coordinate holistic care.

Community Outreach: Participate in outreach activities and community education programs to raise awareness of social issues and services available.

Crisis Intervention: Respond to emergency situations and provide immediate support and guidance to individuals or families in crisis.

Report Writing: Prepare and present reports, assessments, and case summaries for internal use and external agencies, when required.

Professional Development: Stay updated on the latest social work practices, policies, and legislation, and attend relevant training or workshops.

Essential: ? Level 7/8 bachelors degree in social work or related field.
