Assessment and Placement Officer, Homeless Action Team, Mid West
Location: Limerick City and County Council/ Limerick Homeless Action Team
Contract: Full-time, 1 Year-Fixed Term
Hours: Monday to Friday 9am to 5pm
*Experience working in a social care setting required*
The purpose of this role is to provide a support service within Limerick City and County Council/ Limerick Homeless Action Team whereby clients can access advice and information regarding their eligibility for emergency homeless services.
What you will do:
- Make and take appointments, interview, and assess clients seeking homeless services by appointment or unscheduled presentations at the public counter in Limerick City and County Council Limerick Homeless Action Team.
- Provide assessment and advice to people who are homeless.
- Participate in the Homeless Action Team as necessary.
- Assist clients in sourcing suitable private rented accommodation.
- Work with clients in emergency facilities to secure long-term accommodation and avoid continued placement in emergency facilities.
- Assist clients in settling into accommodation and ensure they are fully briefed on available services from external agencies.
- Receive and act upon referrals via the interdisciplinary referral protocol.
- Assist eligible persons for social housing by providing information and assistance in presenting applications.
- Investigate cases thoroughly, maintaining links with authorities, Gardai, CWO's, etc.
- Increase access to health and social services for those experiencing homelessness.
- Create and maintain client files and records.
- Provide the Coordinator with monthly reports on caseload and move-on plans.
- Work with a client-centered approach, encouraging autonomy and independence.
- Establish links with local services and community groups, working closely with housing providers where appropriate.
- Manage an allocated caseload, adhering to Care and Case Management protocols.
- Respond appropriately to child protection and welfare concerns.
- Liaise and collaborate effectively with team members and in relation to organizational policies.
- Comply with all relevant policies, including Child Protection, Confidentiality, Health and Safety.
- Maintain accurate records and statistics per policy.
- Be vigilant to health, safety, and welfare risks and report concerns.
- Attend team and agency meetings, participate in training, and undertake other duties as assigned.
What you will need:
- A relevant third-level qualification in social care, psychology, sociology, etc.
- Experience in a social care setting, including dealing with child protection and welfare.
- Experience in assessments, support plans, and reviews.
- Ability to handle challenging behavior, work independently and in a team, and demonstrate flexibility.
- Willingness to develop professional skills.
- Desirable: advocacy, report writing, IT skills.
