Medical Social Worker - Staff Grade

(Full Time, Part Time, Temporary, Permanent)

We invite applications from suitably qualified individuals for positions on full-time, part-time, permanent, and temporary panels.

We are looking for a dynamic, enthusiastic, and driven Medical Social Worker to provide a quality social work service to patients of the National Rehabilitation Hospital, as well as their families and carers. The role involves working as part of an inter-disciplinary team, playing a key role in setting goals, planning treatment, and coordinating patient discharge, ensuring families and carers are fully integrated into every stage of the process.

Requirements

  • CQSW / NQSW or equivalent qualification
  • Valid registration with CORU or the ability to register by Summer 2025
  • Previous experience in disability services, rehabilitation, or hospital settings
  • Experience in acquired neurological conditions is desirable
  • Full right to work in Ireland

Responsibilities include:

  • Conduct psycho-social assessments of patients and families/carers regarding illness/disability management
  • Participate in goal setting and treatment planning with patients, families, and the Interdisciplinary Team (IDT)
  • Facilitate communication between patients, families, and the IDT to improve continuity of care
  • Provide counselling and psychosocial support through individual, family, and group approaches
  • Participate in educational programmes for patients and families
  • Attend clinical meetings, case conferences, and multidisciplinary team (MDT) meetings
  • Lead family meetings with IDT members
  • Engage in discharge planning from admission, coordinating care and ensuring patient/family involvement
  • Liaise with community and other agencies to access support services and entitlements
  • Organise community case conferences for care planning and referrals
