Job Overview: Outreach Social Care Worker
We are seeking a compassionate and dedicated Outreach Social Care Worker to join our team. The Outreach Social Care worker will provide supports within the framework of New Directions, an outcomes-based individualised approach to services for people with intellectual disabilities. The purpose of the role is to support individual/s in an outreach service to access a wide-range of community-based services and opportunities to meet their needs and interests.

In this role, you will support individuals with intellectual disabilities in accessing a wide range of community-based services and opportunities, helping them achieve their personal goals and fostering their integration into the local community.

- Person-Centred Planning & Care: Develop and manage individual support plans that align with the personal goals, preferences, and needs of individuals. Actively involve them in decision-making using accessible communication tools.
- Community Integration: Establish and encourage partnerships within the community to enhance opportunities for social and vocational engagement. Promote inclusivity and foster a sense of belonging.
- Advocacy and Empowerment: Advocate on behalf of individuals to access needed services and resources, overcoming barriers related to language, culture, and other factors. Empower individuals to take an active role in their care planning.
- Risk Assessment: Regularly conduct risk assessments to identify potential hazards, document findings, and implement appropriate safety measures. Educate and support individuals in recognizing and reporting potential risks.
- Safeguarding: Identify and respond to indicators of abuse or neglect, adhere to safeguarding protocols, and collaborate with multidisciplinary teams to ensure a safe environment for all individuals.
- Health & Safety: Ensure compliance with Health & Safety legislation and organisational policies, manage incidents effectively, and maintain a safe working environment.
- Behaviours of Concern: Address and manage behaviours of concern using appropriate interventions, de-escalation techniques, and crisis intervention strategies.
- Team Collaboration: Work closely with team members, families, and other professionals to provide comprehensive support, maintain excellent communication, and contribute to a positive team environment.
- Administration: Maintain accurate records, prepare timely reports, and adhere to best practices in record-keeping and communication.
- Hold a QQI Level 7 qualification in Social Care, Applied Social Studies, or a related field.
- Minimum of one year's post-qualification experience in the intellectual disability sector, including experience with individuals with Autism and behavioural supports.
- Eligibility for registration with relevant professional body CORU in Ireland.
- Possession of a valid driver's license and willingness to transport individuals using company vehicles.
- Proficient in english language both oral and written
- Have the legal right to work in Ireland and currently hold valid work authorisation for Ireland
- Ability to speak Ukrainian and/or Russian.
- Management qualification.

- Good IT and administrative skills.
