Responsibilities
  • Conduct assessments to identify client needs and develop tailored care plans.
  • Provide counseling and support to individuals and families dealing with various challenges.
  • Collaborate with multidisciplinary teams, including healthcare providers, to ensure comprehensive care.
  • Advocate for clients' rights and access to necessary services and resources.
  • Maintain accurate and confidential client records.
  • Offer crisis intervention and emergency support as needed.
  • Facilitate group sessions and workshops on relevant social issues.
Requirements
  • Bachelor's degree in Social Work or equivalent
  • Professional registration with CORU
  • Proven experience in primary care or similar healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Excellent problem-solving and crisis management abilities.
  • Knowledge of local community resources and services.
  • Empathy, patience, and a non-judgmental approach.
  • Strong organizational skills and attention to detail.
  • Right to work in Ireland without restriction.
  • All candidates will be required to undergo Garda Vetting and International Police Clearance if applicable.
