This is an excellent opportunity for an experienced, motivated, solution focused team player with a deep understanding of the complexities associated with neuro-physical disabilities. The Crann Centre’s Model of Care delivers a range of services and supports- blending a learning and supportive care environment with a welcoming family-centred ethos.

This role will provide direct support to the Head of Services with key responsibility for the management of the client liaison team and collaboration with the services team to ensure that Crann’s clients and families goals and needs are met under Crann’s 2Gen Model of Care .

MAJOR DUTIES & RESPONSIBILITIES:

  • Direct management of the client liaison team with dotted line responsibility for the Programme Management team.
  • Help build and support an efficient and effective team, identifying and addressing staff development needs.

2) Client Referral and Waiting List Management:

  • Oversee new client referrals and monitor client and family needs.
  • Support the Head of services to develop and establish referral pathways.
  • Ensure timely identification and delivery of services, actively managing waiting lists.

3) Intake Process:

  • Regularly review intake processes, assessment forms, and metrics for service delivery efficiency.
  • Support the client liaison team in maintaining consistency in intake assessments, enrollments, and follow-up.

4) Client Caseload Management:

  • Assign a dedicated point of contact for clients within the services team.
  • Overall responsibility for client intake assessments, service plans, referrals, and follow-up processes.

5) Audit, Research, Impact, and Governance:

  • Contribute to data collection, analysis, and reporting.
  • Support Impact Measurement & Management initiatives.
  • Assist in the development and review of PPPGs, care plans, and risk assessments.

6) Service Planning:

  • Contribute to service planning, providing reports on demands, timeframes, referral numbers, and resource requirements.

7) Data Management:

  • Maintain updated and confidential records, ensuring accurate input into Crann’s databases.

8) People Management:

  • Build and support a highly effective team.
  • Help identify staff CPD requirements and contribute to training and development as needed.
  • Support the services team to achieve their objectives and KPI’ s are met.
  • Contribute to the culture and values of the Crann Centre and support engagement across the wider Crann Centre team.

9) Reporting:

  • Provide regular reports to the Head of Services on Key Performance Metrics.
  • Identify and communicate any delays in service delivery, client concerns, and service needs.
  • Ensure appropriate feedback and updates to community and clinical/community health professionals on referrals.

Essential Knowledge, Skills and Experience:

  • Essential: 5-8+ years’ experience working in the healthcare sector with at least 5 years working in/across disability sector.
  • Educated to Degree level or equivalent within nursing, clinical, social work or healthcare background.
  • Strong track record of Continuing Professional Development, 5-10 years’ experience and track record in project management, leadership, experience at leading this type of a team.
  • Leadership – capacity to lead a group of professionals towards a shared common vision / goal.
  • Experience in Impact Management & Measurement to key stakeholders clear findings evidence-based (collection and analysis of impact data)
  • Excellent communication and interpersonal skills.
  • Ability to build and maintain relationships.
  • Committed to quality service and have a strong attention to detail.
  • Team player and self-motivated problem solver.
  • Ability to work under pressure and meet deadlines.
  • Comfortable in a fast-moving environment.
  • Ability to demonstrate integrity and ethical practice.
  • Be a self-starter, with initiative, energy, commitment.
