CDNT Community Liaison Nurse Permanent Contract 18.75 hours per week South Kildare West Wicklow Childrens Disability Network Team (CDNT) Clinical Nurse Manager 2 Salary Scale Max Point 1: €29,830 (pro-rated for 0.5 WTE) In line with the HSE National Programme for Progressing Disability Services , Kare is the Lead Agency for the provision of supports across South Kildare West Wicklow. The successful candidate will join an established Interdisciplinary CDNT, which reconfigured in 2014, providing quality, evidenced based and family-centred supports for children and young people aged 0-18 years, who present with complex needs associated with disability, within an interdisciplinary framework, to support their development to enable active participation in community life. The Community Liaison Nurse will support children who present with complex needs associated with a disability on Kares Childrens Disability Network Team (CDNT) and will work as a key member of the interdisciplinary team providing a family centred quality service to children and their families in line with the National Programme for Progressing Disability Services for Children and Young people. Responsibilities Manage client care to ensure the highest professional standards using an evidence based, care planning approach Provide a high level of professional and clinical leadership and practise Provide safe, comprehensive nursing support to clients with complex disabilities where required according to the Code of Professional Conduct as laid down by the Bord Altranais (NMBI) and Professional Clinical Guidelines Practice nursing according to: Professional Clinical Guidelines, National and Area Health Service Executive (HSE) guidelines, Local policies, protocols and guidelines, and Current legislation Manage own caseload in accordance with the needs of the post and participate in the referral/screening process Participate in teams/meetings/committees as appropriate, communicating and working in cooperation with other team members Collaborate with children and young people, family, carers and other staff in treatment/care planning and in the provision of support and advice To innovate care and initiate new policies, procedures and guidelines using evidence based practice where possible and developing an evidence base where minimal information may exist To monitor and evaluate clinical outcomes of patient centred care including the Integrated Family Support Plan Provide support to children & their families in preparation for major transitions commencing pre-school, school, special school, going for surgery, and transfer to another team etc. Foster good working relationships with Local & National acute paediatric services and Community Paediatrician Have a working knowledge of HIQA Standards as they apply to the role and comply with associated HSE protocols for implementing and maintaining these standards as appropriate to the role This list is not exhaustive. Requirements Be currently registered on the Register of Nurses maintained by Bord Altranais (NMBI) in the Intellectual Disability Nurses Division or Childrens Nurses Division, And Have at least 5 years post registration experience, of which 2 must be in the speciality or related area of Community Nursing, or Paediatric Disability, with knowledge and experience in child development, And Have the clinical, managerial and administrative capacity to properly discharge the functions of the role, And Demonstrate evidence of continuing professional development at the appropriate level Maintain annual registration with the NMBI Demonstrate depth and breadth of nursing experience in the area of Children with complex disabilities Excellent communication, interpersonal, teamwork and effective caseload management skills Awareness of relevant legislation and policies Ability to set up clinical standards of care and ensure they are appropriately audited Full driving license with access to own transport is essential Benefits include: Generous days annual leave Paid Maternity/Paternity Leave (subject to service) Rewarding work with flexible hours Employee Assistance Programme Contributory Pension Plan Optional Salary Protection Plan Personal Development, Professional Growth and Career Progression A detailed Job Description is available on request.
