Position: Qualified Social Worker - Adult Primary Care Location: Portarlington / Mountmellick, Co. Excel recruitment are recruiting a Qualified Social Worker to join an Adult Primary Care team in the Laois area for an agency contract. This role offers an excellent opportunity to make a real difference Key Responsibilities: Providing comprehensive social work assessments for adults with a wide range of social, emotional, medical, and environmental needs. Supporting service users to access appropriate community services, supports, and care pathways. Developing person-centred intervention plans, promoting autonomy, safety, and improved quality of life. Working collaboratively with the multidisciplinary Primary Care Team including nursing, physiotherapy, occupational therapy, psychology, and other allied health professionals. Advocating for service users, ensuring their rights, dignity, and choices are respected. Engaging with families, carers, and external agencies to coordinate effective support networks. Maintaining accurate and timely documentation, reports, and case notes in line with HSE standards. Essential Criteria: CORU-registered Social Worker (essential). Experience working within Adult Primary Care, community services, medical social work, or a related area. Strong knowledge of community resources, social supports, and HSE care pathways for adults. Excellent communication, assessment, and advocacy skills. Ability to manage a varied caseload and work both independently and as part of a multidisciplinary team. Full clean driving licence and access to transport (essential).
