Disability Liaison Worker (Outreach)

Hours:

1 Full-Time Position: 35 hours per week

Location:

Based at Wellsprings Outreach Service, Bethany, Sharman Crawford Street, Cork, with additional community outreach as required.

Service Overview:

Wellsprings offers a supportive outreach service to young women aged 16–23, including those with disabilities, who are transitioning from care or are at risk of homelessness. Our outreach services aim to support personal development, independent living, and community integration. These new Disability Liaison Worker roles aim to ensure that the needs of young people with disabilities are specifically addressed in an inclusive, empowering, and targeted manner.

Purpose of the Role:

To provide tailored outreach support to young people with disabilities, ensuring their full inclusion and access to services, while also advocating for their rights and needs within community and service settings. The role involves one-to-one casework, liaising with families and agencies, group facilitation, and ensuring compliance with relevant disability and equality legislation.

Key Responsibilities:1. Disability-Focused Outreach Support

· Work directly with young people with disabilities to support their transition to independent or supported living.

· Develop and implement individual support plans in line with each person's goals and needs.

· Provide regular outreach visits and maintain consistent contact with young people in the community.

2. Advocacy and Access to Services

· Advocate on behalf of young people to ensure access to education, training, healthcare, and social services.

· Work collaboratively with families, care teams, schools, and disability services to support the individual’s needs.

· Liaise with relevant statutory and voluntary agencies regarding entitlements and supports.

3. Programme Development and Group Work

· Plan and deliver inclusive group sessions promoting independent living, social skills, and community participation.

· Develop accessible materials and adjust programmes to accommodate a variety of needs.

· Support group outings, skill-building activities, and peer support opportunities.

4. Monitoring and Documentation

· Maintain clear, accurate, and confidential case notes and progress reports.

· Contribute to internal monitoring and external reporting as required by funders or governing bodies.

· Participate in case reviews and service evaluations with the wider team.

5. Inclusive Practice and Service Development

· Support the development of a more inclusive outreach programme across Wellsprings services.

· Promote the voice and participation of young people with disabilities in service planning.

· Stay informed on developments in disability rights, legislation, and best practice.

Professional Development

- Engage in regular supervision and reflective practice.

- Attend relevant in-service and external training opportunities, particularly in the area of disability and inclusion.

- Maintain an up-to-date understanding of relevant disability rights and policies.

General Duties

- Adhere to all Wellsprings policies and procedures, including those related to Child Protection, Health and Safety, Equality and Inclusion, and Data Protection.

- Respond to the needs and concerns of young people and their families with empathy and professionalism.

- Carry out additional duties as required to support the aims of the Outreach Service and the organisation.

Reporting Line:

Reports to the Outreach Coordinator and the Manager of the Service.

Remuneration:

Remuneration is aligned with the HSE Social Care Worker scale and will reflect qualifications and experience. These positions are subject to available funding.
