Job Title: Anaesthetic Registrar Department: NCHD's Job Type: Permanent Hours: 39 per week (Full-Time) The role of the Anaesthetic Registrars is to support the provision of anaesthesia, facilitating safe and efficient patient care on a twenty-four hour basis. The attending consultant retains individual responsibility for the patient at all time and the anaesthetic registrars are at no time a substitute for consultant presence. KEY RESPONSIBILITIES Main activities The anaesthetic registrar will at all time work under the direction of a consultant anaesthetist on the medical society at Blackrock Clinic Ensure all documentation is maintained in accordance with hospital policies. Liaise in an effective and open way with all other team members and consultants topromote a team approach to support. Work within the quality system framework to meet with regulation and accreditation standards. To Attend Cardiac Arrest in the Hospital. Carry out assessment intraoperative care and post operative procedures. Carry out ward rounds and participate in daily clinical duties Participate in Clinical Audit and present at least one audit during their attachment. Cardiac Anaesthetic Registrar Provides a daily provision of cardiac anaesthesia services under the direct supervision of the attending consultant The registrar should report each morning to the consultant to establish a plan for the day, assist in medication preparation and be present for induction of anaesthesia. The registrar should assist in acquisition of echocardiography images and their interpretation. The registrar should assist the consultant to ensure safe transition to cardiopulmonary bypass and subsequently separation from cardiopulmonary bypass. The registrar should assist the consultant in the stabilisation of the patient post CPB, and the transfer of the patient to the Intensive Care Unit. Ultimate responsibility for all aspects of patient care lies with the consultant The consultant should provide on-site training and education to the registrar as appropriate. Any issues around clinical competency and performance of the registrar should be brought to the attention of the clinical lead in Cardiac Anaesthesia and then onward (if deemed appropriate) to the Director of Anaesthesia. The Director of Anaesthesia should meet formally with the Cardiac registrar at least once every 3 months to resolve any relevant issues. ICU Anaesthetic Registrar Provides a constant presence and works under the direct supervision of both the attending general and intensive care consultant. The registrar should receive a detailed morning handover from the previous nights registrar, usually at 7.00. They should participate in the morning ward rounds led by the general and cardiac intensive care consultants. They should liaise with nursing staff and establish a working plan for the day. Clinical duties include: assessment and care of patients requiring ICU admission on the wards/ED; provision of expert airway management; provision of expert vascular access; transport of ICU patients for diagnostic testing; and transfer of patients from Hermitage Clinic to the ICU where necessary. The registrar should liaise with the covering ICU consultants, nursing staff,the admitting physician or surgeon, and patient families in an open and coherent fashion. The ICU registrar should be based primarily in Intensive Care and available to attend outside emergency events as required. Audit responsibilities include: attending both weekly consultant and nursing ICU meetings and the monthly MDT meeting; preparing charts for discussion at MDT meetings; engaging in audit activity as directed by the ICU staff; and recording all deaths and critical incidents in the unit. The ICU registrar should keep a log book of the clinical duties undertaken in the course of their working day both as a record of their clinical experience for training purposes and to monitor the range of clinical responsibilities necessary in the post. Handover and ward round sessions should be recorded as should any patient interaction (radiology, theatre, emergency department etc.) The ICU registrar does not have a duty of care to theatre other than in exceptional circumstances and should not be asked to undertake the care of a patient in theatre except in exceptional circumstances.All such instances should be logged. The duty of care for ICU patients ultimately rests with the supervising consultant. The consultant should provide on-site training and education to the registrar as appropriate. The Director of Anaesthesia should meet formally with the ICU registrar at least once every 3 months to resolve any relevant issues. Anaesthetic Registrar On-Call The on-call anaesthetic registrar is involved in the out of hours provision of ICU care and emergency caseload including cardiac anaesthesia emergencies - under the direct supervision of the attending consultant. The consultant to known the on-call registrar reports is the ICU consultant, the cardiac anaesthetic consultant, the general anaesthetic consultant on-call or a consultant involved in the care of a Clinic patient on-call. The registrar should receive a handover from the ICU registrar or the previous on-call registrar on the commencement of their shift to be clinically informed about all potential issues and patients.This should include, but not be limited to, the ICU patient population, any sick patients in the Clinic and potential theatre cases and transfers to/from the ICU. The registrar should carry the emergency pager and respond to all calls The registrar should assist the consultant in the stabilisation of any patients, theatre case management and other clinical issues as directed. Communication Skills Report relevant information verbally and in writing to appropriate personnel. Respond appropriately to customer/colleague requests and questions. Communicate appropriately with colleagues and other members of the health care team. Recognise self-limitations and seek assistance. Attend and contribute in staff meetings and team briefing. Communicate in a manner that avoids antagonism, reduces conflict and prevents undue anxiety. To attend and contribute to appropriate group meetings with other hospitals. Health and Safety Comply with Health and Safety policies and procedures which includes ensuring equipment used on the service is maintained to the required standard and defects are reported to appropriate authorities as soon as possible. Implement agreed policies, procedures and safe professional practice. Work in a safe manner with due care and attention to the safety of self and others. Be aware of Risk Management issues, identify risks and take appropriate action. Report any adverse incidents or near misses. Adhere to department policies in relation to the Care and Safety of any equipment. Administration Actively participate in the improvement and development of the services by liaising with the appropriate personnel Carry out other duties appropriate to the post as required from time to time by the Consultant Cardiac Anaesthetist. Professional Expectations Function within the limits of the hospitals policies, procedures and standards. Support the Mission Statement of Blackrock Clinic. Observe appropriate lines of authority. Demonstrate flexibility by assisting in other areas of the hospital as required. Set annual goals and objectives with the Consultant Anaesthetist Continue to develop personal knowledge and skills. Provide direct supervision to other members of the team when appropriate. Participate in working groups to develop and promote quality service within Blackrock Clinic Participate in the education of learners and assist with the orientation of new personnel.
