This appointment will be on the basis of grade-to-grade reassignment/secondment and current remuneration will apply.

Terms and ConditionsThe successful candidate will retain all current terms and conditions (including remuneration and annual leave) on reassignment/secondment into the role.

The standard working week associated with this post is 0.5 WTE per week, Monday to Friday. (Specific working days to be agreed on appointment). Clinical Practice will be facilitated.

Reporting Relationships:The successful post holder will report to the NCAGL Office for Older Persons. The Clinical Lead should keep the programmes Clinical Advisory Group (CAG) abreast of their programme of work and progress of this work at regular intervals and no less than a quarterly basis.

Location of Post: The successful candidate will maintain their existing base/location of work and will be expected to be available for meetings as required at other locations.

Details of Service

The Office of the Chief Clinical Officer (CCO) was established as part of an overall investment by the HSE to strengthen governance and accountability for the planning and delivery of high-quality services with the aim of driving transformational change across our healthcare system through clinical leadership, design of new models of care, promotion of a culture of safety and quality improvements through patient and service user involvement.

The Clinical Design and Innovation (CDI) function, reporting to the National Clinical Director of Integrated Care, within the Office of the CCO provides Clinical Innovation, Design Leadership and Direction to strategic decisions across the wider organisation.

The core role of the Office of the NCAGL for Older Persons is to provide clinical leadership and guidance to effect the development and implementation of best practice care pathways and models of care and a coordinated approach to integrated, patient-centred care that is sustainable into the future.

Details of the Programme The Rehabilitation Model of Care for Specialist Rehabilitation Medicine was launched in 2018 with a key priority and focus on the development and implementation of a Hub and Spoke Model of Care (MOC) for specialist rehabilitation through Managed Clinical Rehabilitation Networks, care pathways and developed through evidence based best practice, relevant science in the field of rehabilitation, and international best practice guidelines.

The National Clinical Programme for Rehabilitation Medicine aims to:

1. Provide national clinical leadership in the design, implementation, and evaluation of specialist rehabilitation services.
2. Ensure equitable access to high quality, evidence-based rehabilitation care for patients across Ireland.
3. Embed patient-centred care pathways that optimise recovery, independence, and quality of life.
4. Support integration of rehabilitation services across acute, post-acute, and community settings within a Hub-and-Spoke framework.
5. Advance service delivery through research, innovation, data-driven evaluation, and international best practice.

The key objectives of the Programme are to:

1. Implement the Model of Care for Specialist Rehabilitation Medicine, including the development of Managed Clinical Rehabilitation Networks.
2. Standardise and modernise patient pathways, ensuring timely referral, access, and continuity of rehabilitation services across the health system.
3. Develop and monitor KPIs and outcome measures to demonstrate effectiveness, safety, and value of rehabilitation services nationally.
4. Strengthen workforce capacity by supporting education, training, and multidisciplinary team development in rehabilitation medicine.
5. Foster integration and collaboration with other National Clinical Programmes (e.g., Stroke, Older People, Paediatrics, Disability), Neurorehabilitation Strategy and National Office for Trauma Services to provide seamless, patient-centred care.
6. Ensure the patient is central in service design and evaluation, working in partnership with patients, families, and advocacy groups.
7. PROMOTE research and innovation in rehabilitation medicine, embedding best practice and new technologies into service delivery.

Reporting RelationshipsThe successful post holder will report to the NCAGL Office for Older Persons. The Clinical Lead should keep the programmes Clinical Advisory Group (CAG) abreast of their programme of work and progress of this work at regular intervals and no less than a quarterly basis.

Key Working RelationshipsThe proper execution of duties will involve the development of appropriate communication arrangements with key stakeholders, both internal and external. These include, but are not limited to:

- National Clinical Director, Integrated Care and team.
- HSE National Directors, National Clinical Advisor and Group Leads, Hospital CEOs, REOs, Clinical Directors, Consultants, GPs, Directors of Nursing, HSCPs HSE National Services, Department of Health, postgraduate training bodies and professional representative groups.
- National Clinical Programme Leads: the appropriate Programme Managers & HSCP Leads, Nurse Leads as well as Management within designated NCAGL office.
- The post holder will work to build relationships across hospital and community with all key stakeholders.
