**Role Summary:**

The successful candidate will work closely with the multi-disciplinary team to provide support services to young people.

**Key Responsibilities:**

  1. Deliver support services to young people, including provision of basic physical care and support.
  2. Establish trusting relationships with young people and facilitate their transition to a more stable lifestyle.
  3. Collaborate with other agencies and services to coordinate support for young people.
  4. Maintain accurate casework records and input information into the organisation's database.

**Requirements:**

  1. A degree-level qualification in social care.
  2. Experience working with disadvantaged young people.
  3. Knowledge of issues affecting young people out-of-home.
  4. Experience managing high-risk situations.
  5. Teamwork experience.
