Disability Support Coordinator

About the Role
We are seeking a highly motivated Disability Support Coordinator to join our team. As a Disability Support Coordinator, you will be responsible for promoting and supporting independent living and community integration for service users. This includes designing and delivering individual programmes and activities that meet the needs, choices, and abilities of our clients.
Key Responsibilities
- Develop and implement programmes and activities that support service users to achieve their goals
- Build trusting relationships with service users and facilitate them to identify their strengths and needs
- Support service users to access community-based supports and services
- Advocate on behalf of and support self-advocacy for service users
Requirements
- Third level graduate in health and social care or equivalent
- MAPA, Autism Awareness Training, Occupational First Aid, and Manual Handling certification
- Experience working with adults with disabilities
- Well-developed people management skills with proven experience of building, leading, and motivating a team
