About the RoleWe are seeking a compassionate and organized individual to join our team as a Person-Centred Care Coordinator. In this role, you will work closely with adults with severe/profound intellectual disabilities and additional support needs to coordinate and deliver person-centred care services.
Main Responsibilities:
- Coordinate and implement person-centred plans to support individuals' goals and aspirations.
- Collaborate with service users, families, and other stakeholders to identify and address their needs.
- Assist with daily living tasks, such as personal care, meal preparation, and household chores.
- Encourage participation in meaningful activities, including recreational, cultural, and social events.
- Develop and maintain positive relationships with service users, families, and other stakeholders.
Requirements:
- QQI Level 5 Certificate in Health Care or equivalent.
- Experience working with adults with intellectual disabilities.
- Full clean manual driver's license required.
- Flexibility around working hours, including evenings and weekends.
