Role Responsibilities:
Deliver high-quality interventions to adults within the Primary Care setting.
Conduct comprehensive assessments and develop individualized care plans to promote functional independence in activities of daily living (ADLs).
Provide intervention in areas such as self-care, mobility, housing adaptations, equipment prescription, and falls prevention.
Work collaboratively as part of a multidisciplinary team, including Physiotherapists, Speech and Language Therapists, Public Health Nurses, and GPs.
Supervise and support junior staff, students, and assistants, ensuring high standards of practice.
Participate in the development and implementation of service improvement initiatives within the Primary Care team.
