Job Title: Community Health Care Professional
">Job DescriptionWe are seeking an experienced and dedicated Community Health Care Professional to provide high-quality, person-centred care to children with life-limiting conditions and their families in the family home/community environment. This is a deeply rewarding role that requires a strong commitment to delivering excellent patient care and support.
Key Responsibilities:- Develop and implement individualised care plans that meet the unique needs of each child and their family.
- Provide high-quality, compassionate care to children with life-limiting conditions and their families.
- Work collaboratively as part of an interdisciplinary team to achieve positive outcomes for patients and their families.
- Contribute to the development and implementation of policies and procedures that promote excellence in patient care.
- Be fully registered with the relevant healthcare authority.
- Have post-registration experience in a paediatric setting, preferably an acute setting.
- Have a working knowledge of information technology.
- Be able to work independently and as part of an interdisciplinary team.
The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively with children, families, and healthcare professionals. They will be passionate about delivering high-quality patient care and committed to ongoing learning and professional development.
