**Job Overview:**
We are recruiting for a highly skilled Nurse to join our rehabilitation centre team. The successful candidate will be responsible for delivering high-quality patient care and contributing to the overall well-being of our residents.
Main Responsibilities:
- Care Delivery:
- Develop and implement individualised care plans in collaboration with multidisciplinary teams.
- Deliver evidence-based care practices, prioritising patient safety and quality outcomes.
- Monitor and evaluate patient progress, making adjustments to care plans as needed.
- Serve as a mentor and leader, guiding junior staff members in their roles.
- Contribute to a positive and supportive work environment, promoting teamwork and open communication.
- Accurately maintain patient records, adhering to regulatory requirements.
- Collaborate with healthcare professionals to ensure seamless transitions and continuity of care.
Required Qualifications:
- Registered General Nurse on the Register of Nurses held by Nursing and Midwifery Board of Ireland (NMBI).
- Post-registration qualification in nursing people with dementia.
- Evidence of continuous professional development.
