Job Description:The ideal candidate will have a strong commitment to delivering compassionate and personalized care to patients with varying levels of need. They will be responsible for assessing, planning, and delivering individualized care plans that cater to the unique needs and goals of each patient.
Key Responsibilities:- Assess patients' physical, emotional, and social needs
- Develop and implement individualized care plans to meet patients' needs
- Collaborate effectively with interdisciplinary teams, including nurses, doctors, therapists, and other healthcare professionals
- Long-stay residential unit with respite and palliative care beds (25 beds)
- Rolling roster with shifts on Monday through Sunday
