About the Opportunity

The successful candidate will facilitate service users in their choices, developing programmes within a person-centred model of service delivery. Programmes may include independent living, personal development, and occupational and leisure activities. We promote independence and community integration.

Key Responsibilities:
  • To act as a key worker for service users to facilitate the Person Centred Planning process.
  • To identify goals and supports required by individual service users and maintain records as necessary.
  • To develop and deliver person-centred programmes to meet the needs of service users as identified through Person Centred Planning, with accurate record-keeping.
Requirements:
  • A valid driving licence is essential for this role.
  • Minimum QQI Level 5 in Social Care or equivalent qualification is required.
