We are seeking a qualified professional to fill the role of Community Support Specialist.
Key Responsibilities:- Collaborate with the multidisciplinary team on individualized care plans
- Assist clients and families with housing applications and related support services
- Advocate for client needs within community and statutory agencies
- Provide emotional and practical support during transitions
Requirements:
- CORU registration as a Social Worker (essential)
- Experience in ABI, disability, or hospital/community social work (desirable)
- Strong communication, advocacy, and team collaboration skills
- Commitment to person-centered care and rehabilitation
