Home Instead Down and Lisburn has an exciting opportunity for a Care Team Leader to join our fantastic Care Team. This role plays a vital part in ensuring that our clients receive high-quality care while supporting the growth of our business.

Job Description

The Care Team Leader will work closely with the Client Experience team to ensure the highest quality of personalised care is delivered. This includes maintaining regular contact with clients and Care Professionals, participating in on-call duties as required, and complying with Home Instead's Equality, Diversity and Equal Opportunities Policy.

Responsibilities

The successful candidate will be responsible for:

  • Supporting care consultations, service reviews, and completing client support plans.
  • Conducting staff supervisions and client Quality Assessments.
  • Creating, updating, and auditing client care plans and assisting with digital care planning.
  • Maintaining accurate client and Care Professional records on Home Instead software.
  • Providing support to the Client Experience team, liaising with schedulers to ensure the highest quality of personalised care is delivered.
  • Participating in on-call duties as required.
  • Complying with Home Instead's Equality, Diversity and Equal Opportunities Policy.
Requirements

To be successful in this role, you will have:

  • A minimum of 12 months experience working in a Health and Social care environment with responsibility for managing a team of people.
  • Good communication skills with the ability to build rapport quickly.
  • Confidence to use care management technology including providing support and training to Care Professionals.
  • A good working knowledge of IT systems with experience of Microsoft Office and virtual communication platforms.
  • A full UK driving licence and means of transport when required to visit clients.
  • Organisation and flexibility to meet the needs of the business.
