Key Responsibilities:

  • Develop and implement individualised plans to promote independent living and community integration.
  • Liaise with families and multidisciplinary teams to ensure seamless coordination of services.
  • Provide support and guidance to service users in accessing local resources and integrating into their community.
  • Maintain accurate records and document information regarding service user interactions and progress.
  • Comply with health and safety legislation and ensure safe use of appliances and equipment.

Requirements:

  • A minimum of one year's experience working in the field of disability is essential.
  • Full driving license and access to a car for business purposes insured in your name are mandatory.
  • Desirable qualifications include a third-level degree in Social Care, although not necessarily.

Benefits: As a valued member of our team, you can expect a competitive salary starting from €29,374.70 (pro rata) with potential for negotiation based on experience and qualifications.
