As a Community Connector, you will play a vital role in our Adult Services team, supporting adults with physical disabilities to build meaningful relationships and engage with their communities.

About the Role

  • You will work closely with our Strive project team to deliver key outcomes for individuals, focusing on independent living and personal development.
  • You will be an integral part of the transition group team, providing support and guidance to service owners as they navigate their journey towards community integration.
  • You will be responsible for developing and implementing Person-Centred Plans, tailored to meet the unique needs and goals of each individual.

Requirements

  • You must hold a minimum qualification of QQI Level 5 Certificate in Social Care or a relevant health-related area, or be in your first year of a relevant degree program.
  • You must have a minimum of one year's experience working with adults with disabilities, preferably in a keyworker role.
  • You must possess a full clean driver's license and be willing to drive Enable Ireland vehicles.
