Job Description:As a key member of our care team, your primary responsibility will be to deliver exceptional support to our service users, ensuring their physical, emotional, and psychological well-being is maintained at all times.
You will work collaboratively with the care team to develop and implement individualized support plans that cater to the unique needs and goals of each service user.
- Provide personal care and support to service users, including assistance with daily living tasks, meal preparation, and medication management.
- Work closely with external agencies, such as healthcare professionals and family members, to ensure seamless coordination of care services.
To succeed in this role, you will require excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and external agencies.
- A degree or diploma in social care, healthcare, or a related field is desirable.
- Experience working in a residential care setting, particularly with individuals with complex needs.
- Knowledge of relevant legislation, policies, and procedures governing care services.
