Job Title: Nurse Care Coordinator

The primary goal of our service is to support individuals with intellectual disabilities to achieve their aspirations and goals. This is accomplished by providing a flexible approach to work and working hours, in line with Personal Outcome Measures.

As a Nurse Care Coordinator, you will be responsible for delivering person-centered care to individuals with intellectual disabilities. Developing strong relationships with families and key individuals involved with the people we support is crucial.

We expect our team members to actively pursue and ensure the delivery of a person-centred service through the achievement of Personal Outcomes. As a Key worker, you will be involved in assisting new staff in this role. You will also be responsible for recording and monitoring outcomes, as well as tracking progress.

The main responsibilities and functions of this role include:

  • Establishing effective working relationships with families and key individuals involved with the people we support.
  • Actively pursuing and ensuring the delivery of a person-centred service through the achievement of Personal Outcomes.
  • Being involved as a Key worker and assisting new staff in this role.
  • Recording and monitoring outcomes, as well as tracking progress.

In addition to the standard nurse duties, the Nurse Care Coordinator role includes practicing nursing according to professional clinical guidelines and the Code of Professional Conduct issued by the Bord Altranais agus Cnáimhseachais na hÉireann (Nursing Midwifery Board Ireland). The enhanced nurse role requires a commitment to ongoing learning and development, ensuring that you remain up-to-date with best practices in the field.

  • The ability to communicate effectively in both written and verbal forms.
  • Creative problem-solving skills.
  • Patience, flexibility, creativity, and a willingness to become involved in a range of activities.

This is a permanent full-time position, requiring flexibility and adaptability. The successful candidate will be required to work weekends, public holidays, and night duty to meet service needs.

The remuneration package for this role includes Department of Health salary scales 01/01/2024, with long service increments after 3 years on the maximum of the scale.
