Job Title: Home Care Coordinator

PE Global Healthcare is seeking a highly skilled and dedicated Home Care Coordinator to join their team. The successful candidate will be responsible for managing and coordinating person-centered care and resources in the home.

Main Duties:
  • Supervise, coordinate, and support the work of home care workers assigned to you.
  • Recruit, induce, and manage staff for the service.
  • Schedule and issue the home care staff roster on a weekly/monthly basis based on individual client needs and cost efficiency.
Client Work:
  • Assess, identify, and manage the needs of potential and existing clients.
  • Produce a detailed specification of the client's home care needs.
  • Review clients on a regular basis by liaising with those responsible for providing care and convey concerns to primary carers/other healthcare professionals.
Administration:
  • Carry out administration required to provide an efficient and effective Home Care Service.
  • Maintain appropriate records of clients receiving home care services.
  • Liaise with primary carers and public health nurses to complete home care records.
Advocacy:
  • Promote awareness of dementia-related issues at a local level.
  • Ensure compliance with the service level agreement by promoting home care hours.
  • Network and liaise with other service providers, health professionals, and potential referral sources to promote the service.
Requirements:
  • A standard education enabling you to discharge the duties of the post satisfactorily.
  • Diploma in Health Service Management or equivalent qualification desirable.
  • Previous experience working in a caring environment, preferably caring for people with dementia, older people, or those with learning disabilities is essential.
  • Excellent communication and interpersonal skills.
  • An understanding of legislation relevant to the role.
  • Full driver's license and access to own vehicle.
