About the Role
">As a Hospital Discharge Worker, you will be responsible for delivering an efficient and effective advice and assessment service to individuals in acute medical services.
">You will work closely with healthcare professionals, local authorities and other stakeholders to prevent or minimise delayed discharge due to homelessness.
">Your key responsibilities will include:
">- Assessing and meeting housing and support needs in line with the homelessness legislation and statutory guidance.
- Collaborating with healthcare professionals, local authorities and other stakeholders to prevent or minimise delayed discharge due to homelessness.
- Referring individuals to appropriate services and supports.
Requirements
">- A minimum of level 7 qualification in Social Care or a relevant discipline.
- A minimum of two years' experience in a Social Care or related field.
- Registered with, or eligible to register with CORU as a Social Care Worker.
