Job DescriptionThis role involves conducting thorough health assessments focused on wound, ostomy, and continence-related care. The assessment includes a physical evaluation, data collection, and analysis to identify potential issues or problems related to wound, ostomy, and/or continence care needs.
The successful candidate will develop and implement personalized care plans to achieve measurable outcomes. They will also collaborate with the healthcare team, provide education to residents and their families, and participate in staff training programs.
Required Skills and Qualifications- Nursing degree from an accredited institution
- Current Missouri nursing license in good standing
- CPR/AED certification
- Wound certification preferred
- Minimum two years of professional nursing experience, preferably in post-acute settings
