Company Overview: Cpl Healthcare is proud to partner with a non-profit homeless service provider in Clare, dedicated to delivering a 'Housing First' approach to individuals experiencing long-term homelessness.

The Role:

  • As a key member of the housing team, you will work closely with clients to deliver a safe and welcoming environment, providing basic physical care and support while respecting consumer choice and self-determination.
  • You will establish trusting relationships with clients who are living on the streets or in temporary accommodation, engaging them in work that enables them to move towards a more stable lifestyle.
  • Undertake assessments and support planning with clients as the lead agent, using the Care and Case Management system and protocols to assist them in progressing towards independent living.

Key Responsibilities:

  • Develop methods and structures to support clients with their individual housing, welfare rights, and entitlements, and facilitate access to move-on accommodation.
  • Assist tenants in creating and sustaining a home, implementing practices in line with tenancy settlement indicators.
  • BUILD AND MAINTAIN A PORTFOLIO OF ACCOMMODATION PROVIDERS IN SOCIAL HOUSING, HOMELESS SECTOR, AND PRIVATE RENTED ACCOMMODATION.
  • Accompany and provide support to clients on appointments to other services, such as doctors, counselors, and court appearances.
  • Maintain a caseload of clients.

Requirements/Experience Needed:

  • Degree in social care, youth work, or related discipline.
  • Knowledge and understanding of homelessness and issues affecting people who are or have been homeless.
  • Ability to plan and implement programs.
  • Report writing abilities and IT skills.
  • Flexibility and communication skills.
  • Full driver's license with access to car (desirable).
  • Experience working with customers who are or have been homeless.
  • Experience working with customers identified as having a dual diagnosis.
  • Experience working and liaising with other agencies.
