We are seeking a dedicated and compassionate care specialist to provide high-quality patient care in community settings.
Key Responsibilities:- Deliver evidence-based nursing care to patients in their homes or community settings.
- Collaborate with multidisciplinary teams to coordinate patient care and achieve optimal outcomes.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Maintain accurate and comprehensive records of patient interactions and interventions.
- NMBI registration
- Right to work in Ireland
- Full compliance on file - GV, References, Health screening etc
