Home Health Case Manager

About the Job:

We are seeking a skilled and compassionate Home Health Case Manager to join our team. The successful candidate will work one-on-one with patients in their home environment, establishing and overseeing all aspects of case-managing patient's care.

Key Responsibilities:
  • Assume responsibility for assigned patient's care from admission through discharge by effectively executing patient's Plan of Care.
  • Collaborate with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
  • Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
  • Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
  • Participate in on-call rotation as reasonably assigned.
Requirements:
  • Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
  • Active State Registered Nurse licensure is required.
  • Proof of current CPR certification and valid Driver's license.
