The Role
We are seeking a highly skilled and experienced Senior Social Worker to join our Young Adult Multidisciplinary Team. The successful candidate will be responsible for carrying a social work caseload and providing a social work service to young adults aged between 17 years 6 months and 29 years 6 months with a disability and their families.
Key Accountabilities
- Develop and maintain effective relationships with young adults, families, and other stakeholders.
- Provide tailored social work support to meet the unique needs of each individual.
- Collaborate with the multidisciplinary team to ensure seamless service delivery.
- Ensure accurate record-keeping and compliance with regulatory requirements.
Desirable Qualities
- Experience working with families and adults with intellectual disabilities in a senior role.
