Job Description:

  • As a Start of Care nurse, you will receive the training and support needed to succeed in delivering exceptional care.

Key Responsibilities:

  • Care for clinically complex patients in their homes and expand your clinical skills.
  • Conduct 1-3 Start of Care visits per day in a designated geographical area, focusing on 1:1 care with each patient.
  • Deliver 1:1 skilled nursing, intervention, and supportive care in accordance with individual care plans, including medication administration, wound care, treatments, and comfort care.

Qualifications:

  • A current NJ nursing license in good standing is required.
  • At least one year of related experience, preferably in Home Health, is necessary.
  • You should be comfortable using an EMR, and experience with HCHB is a plus.
