Mid West Community Healthcare is committed to delivering high-quality primary care services to its patients and communities. In response to a need identified by the community, we are seeking a Social Care Leader to join our team.

The purpose of this role is to ensure that the experiences, preferences, and needs of patients and service users across all socio-demographics within the Community Healthcare Network (CHN) are heard and responded to.

To achieve this, the post holder will work in partnership with the Primary Care Team across the CHN to continue taking an inclusive population-based approach to delivering primary care services with an emphasis on marginalized communities within North Limerick City.

This role involves working closely with other functions providing support to the population of the Network, such as Social Inclusion, Mental Health, Disabilities, and the Office of Patient Partnership.

The successful candidate will be responsible for understanding the barriers identified through comprehensive engagement completed through the North Side Engagement Project and implementing key recommendations co-produced within the Network to address these barriers.
