About The Role
The Support Worker will be part of an interdisciplinary team delivering a quality service to children and their families.
Key responsibilities include reviewing clinical reports, documenting outcomes, and recommending referrals under the supervision of the Children's Disability Network Manager. Additionally, they will assist with client discharge, file reviews, and preparation of clinical note summaries. A high standard of documentation is expected, in accordance with local guidelines and legislative requirements.

Required Skills and Qualifications
This role requires a QQI Level 5 Award in Health Care Support / Social Care / Special Needs Assisting or equivalent. Relevant experience supporting children or individuals with disabilities is essential. Understanding of person-centred care and rights-based approaches is also necessary.
