Job Role

We are seeking an experienced Disability Care Coordinator to join our team in Shannon. In this role, you will work closely with our clients and their families to coordinate and deliver support services that promote their independence, dignity, and well-being.

As a Disability Care Coordinator, you will be responsible for developing and implementing personalized support plans in collaboration with the multidisciplinary team. You will also assist clients with daily living activities, such as personal care, meal preparation, and medication management, while promoting community participation and social integration.

To be successful in this role, you will possess excellent communication and interpersonal skills, with the ability to build strong relationships with clients, colleagues, and families. You will also be flexible, adaptable, and able to work effectively in a fast-paced environment.

Key Responsibilities
  • Develop and implement personalized support plans in collaboration with the multidisciplinary team.
