Job Overview

This role is designed to promote and support independent living and community integration by delivering person-centred programmes that cater to the needs of service users.

Main Responsibilities:
  • Develop, implement, and maintain programmes tailored to meet the specific needs of individuals as identified through Person Centred Planning.
  • Act as a key worker to facilitate the Person Centred Planning process for assigned service users.
  • Collaborate with service users to identify their goals and required supports, maintaining accurate records.
  • Promote community access for individuals and groups by facilitating activities and programmes, ensuring seamless integration into community life.
  • Maintain adherence to all rules, regulations, policies, and procedures governing RehabCare services.
Requirements:
  • Hold a minimum QQI Level 5 in Social Care or equivalent qualification.
  • Possess good knowledge of New Directions and community-based programmes.
  • Demonstrate understanding of intellectual disabilities and mental health issues.
  • Exhibit a minimum of one year's experience working in a relevant setting, with a proven track record of facilitating and implementing person-centred plans.
