We are seeking a skilled and dedicated Nurse Care Specialist to provide exceptional care to individuals with intellectual disabilities. The ideal candidate will work collaboratively with families, key workers, and multidisciplinary teams to deliver person-centered services.
Main Responsibilities:
- Establish strong relationships with families and key workers to ensure effective support for individuals.
- Pursue and deliver personalized services through Personal Outcomes.
- Monitor and record outcomes, tracking progress regularly.
- Support the implementation of National Interim New Directions Standards.
- Maintain high standards of hygiene and infection control.
- Ensure confidentiality in all matters concerning individuals supported.
- Promote dignity at work and respect towards individuals, colleagues, and stakeholders.
- Contribute to Team Based Performance Management Programme.
Enhanced Nursing Role:
- Practice nursing according to the Code of Professional Conduct issued by the Nursing Midwifery Board Ireland.
- Work within your scope of practice and maintain necessary competence for professional practice.
- Comply with and adhere to relevant policies and procedures.
- Promote safety, health, welfare, and social wellbeing of individuals supported.
- Maintain accurate written/electronic records, person-centered plans, and care plans.
- Utilize evidence-based screening tools and risk prediction models to support individuals.
- SUPPORT AND COOPERATE WITH THE SHARING OF TASKS AS PER NATIONAL AGREEMENTS.
- Engage in continuous skill acquisition and contribute to learning and development of the wider nursing team.
- Complete mandatory training as deemed necessary.
- Support training and education of nursing students and participate in clinical induction of new staff.
- Contribute to identification of training needs pertinent to the clinical area.
- Develop clinical teaching skills and participate in planning and implementation of orientation, training, and teaching programs.
