Key Responsibilities:
- Coordinating the care and support of young individuals, including developing personalized care plans and monitoring progress.
- Working closely with external agencies to ensure that the needs of young individuals are met.
- Providing support to young individuals who require additional assistance, such as those who have undergone trauma or exhibit challenging behaviour.
Requirements:
- A Third level Degree in Social Care or equivalent.
- Demonstrated experience in a social care environment, preferably in a coordinator role.
- Knowledge and understanding of Child Protection.
