  3. Actively support service users in their daily lives and choices.
  4. Ensure all files are up-to-date and ready for inspection at any time.
  5. Build relationships with families, clinicians, and the community.
  6. Develop and implement positive support programs that promote independence and community integration.
  7. Stay informed on current social care policies and sectoral and national changes related to disability services.
  8. Support service users to engage in planned activities of their choice.
  9. Take ownership of key working in consultation with the service user, encouraging them to participate in meaningful activities.
  10. Provide personal care and support the care of the service user such as everyday needs.
  11. Evaluate and improve your skills and knowledge through reflective practice and learning from service users.
  12. Collaborate with the team to develop strategies and tactics for meeting the ongoing requirements of those being supported.
  13. Maintain confidentiality and uphold the organisation's and Service Users' privacy at all times.
  14. Undertake Health and Safety checks and Risk Assessments, recording and reporting results effectively.
  15. Liaise with multi-disciplinary teams, families, and other relevant people pertaining to the service user.
  16. Attend Person Centred Planning meetings and contribute to discussions regarding service users.
  17. Be adaptable and willing to work shifts to meet the needs of the service.
