Key Responsibilities:
• Deliver person-centered care and support to individuals with complex needs
• Work collaboratively with the multidisciplinary team to develop and implement Individual Plans
• Support service users to participate in meaningful activities and develop valued social roles
• Advocate on behalf of service users regarding their needs and preferences
• Develop and maintain positive relationships with service users, families, and other stakeholders
Requirements:
• Experience in facilitating and supporting individuals with complex needs
• Strong understanding of person-centered care principles and values
