
Care Coordination Specialist

We are seeking a highly skilled Care Coordination Specialist to join our team. This role is responsible for coordinating client scheduling, staff coordination, and compliance.
Key Responsibilities:- Client Scheduling: Coordinating homecare nurse and caregiver placements to meet client needs.
- Scheduling Liaison: Scheduling and roster information liaison with clients, family, nurses, and caregivers to ensure seamless care delivery.
- Booking Coordination: Filling open bookings in a timely manner and coordinating with homecare workers and clients to resolve any issues.
- Client Feedback: Maintaining regular contact with clients to gather feedback on scheduling and services provided.
- Employee Support: Assisting employee queries and resolving them promptly to ensure high staff satisfaction.
- Compliance: Ensuring staff comply with company standards before placement to maintain quality of care.
- Payroll Administration: Following payroll policies and procedures to ensure accurate payments.
- Experience: Minimum 1-2 years' experience in a similar position or related field.
- Communication Skills: Strong written and oral communication skills to interact effectively with clients, staff, and stakeholders.
- Attention to Detail: Attention to detail and proficiency in basic IT systems to manage client data and schedules.
- Interpersonal Skills: Excellent interpersonal and communication skills to build strong relationships with clients and staff.
