Service User Support Specialist

The Rehab Group is seeking a skilled and compassionate individual to fill the position of Service User Support Specialist. As a key member of our team, you will be responsible for facilitating person centred planning processes, developing and delivering programmes to meet the unique needs of our service users.
Your primary focus will be on supporting individuals in accessing their community, facilitating activities and programmes, and ensuring that they receive the highest standard of service. You will work closely with our team members, external agencies, and families to ensure seamless coordination and effective support.
To succeed in this role, you must have a minimum of 1 year's experience working in a relevant setting, with a proven track record of facilitating and implementing person centred plans. A strong understanding of independent living skills, community integration, and programme development is essential.
In addition to your technical skills, you must possess excellent communication and interpersonal skills, with the ability to build rapport with our service users and their families. Your flexibility and adaptability will be crucial in meeting the operational needs of our service.
