Job Title: Care Coordinator

  • To provide a supportive and nurturing environment for individuals with intellectual disabilities.
Key Responsibilities:
  • Foster self-care skills in personal hygiene, health matters, and appearance maintenance.
  • Develop participation in social activities, sports, and leisure pursuits within the residential setting and community.
  • Provide variety and individuality in lifestyle to enable clients to pursue their hobbies and interests.
  • Evaluate, plan, and implement nursing care as appropriate.
  • Ensure religious needs are met and facilitate attendance at training, work, or activation programs.
  • Supervise and direct staff and students, acting as a preceptor to student nurses.
  • Manage household duties, account for private monies, and maintain records of client finances.
  • Devise and participate in implementing Personal Lifestyle Plans, Training Programs, and other relevant plans.
  • Maintain records, issue reports, and administer medication according to policies and guidelines.
Required Skills and Qualifications:
  • Recognized qualification in nursing (RNID desirable) and current registration with An Bord Altranais.
  • Excellent IT, administration, report writing, and organizational skills.
  • Good interpersonal and teamwork skills, with initiative and flexibility.
  • Possess a good knowledge of intellectual disability and HIQA guidelines.
